A few notes on how to use this page:

When you have uploaded your photo, click on the orange “Submit” button.
Once you receive the success message, click the blue “Mark Complete” button.
If you return to this page later, you will see a fresh form. As long as you received a success message previously, we have your original submission.

This photo will appear in the Health Rosetta Advisor directory.
